Context
Doctors of the World (MdM) is an international solidarity organization whose mission is to guarantee universal and sustainable access to quality healthcare, based on the principles of “Treat, Witness, Advocate.” Present in Madagascar since 1986, MdM has strengthened its community-based approaches and its joint work with civil society and health authorities over the years.
Since April 2022, MdM, in partnership with four implementing partners (AFSA, SISAL, FIVEMIMAD, and ORMS-LGBT), has been implementing the Rights, Empowerment, and Health for Sex Workers (DESPS) project along the Antananarivo–Moramanga–Brickaville–Toamasina axis. The project is funded by the French Development Agency (AFD) and Expertise France.
Brief overview of the project
The overall objective of this project is to contribute to improving the health and rights of people with disabilities along the Antananarivo/Tamatave road axis.
The specific objectives and expected results are:
Specific objective 1 : Within 3 years, improve access for healthcare professionals to a quality basic care package, and in particular HIV and STI prevention services included in sexual and reproductive health services, through strengthening the quality of service supply and demand.
Result 1: The provision of prevention and care services in sexual and reproductive health is improved.
Result 2: Demand for SRR care services for sex workers is increased thanks to better knowledge of their rights and the services available in health facilities.
Specific objective 2 : To strengthen the empowerment of the direct beneficiaries of the project by developing an integrated gender and rights approach while combating all forms of gender-based violence.
Result 1: Gender-based violence against sex workers is documented and made visible, and survivors receive better support.
Result 2 : The individual and collective capacities of sex professionals are strengthened to assert their rights and develop new skills and activities.
Result 3 : Community associations are equipped to participate in national debates promoting the application of laws and the recognition of the rights of sex workers.
Result 4 : Implementing partners are strengthened. As part of the project’s monitoring and evaluation framework, a baseline survey was conducted in 2023, followed by a midline evaluation in 2024.
As the project nears completion, MdM-F wishes to conduct an endline survey to rigorously measure changes observed between the baseline and the end of project implementation, and to meet accountability, learning and future planning requirements.
Objectives of the consultancy
General objective
Conduct a comparative endline survey to measure changes between baseline (2023) and the end of the DESPS project, in relation to access to health services, knowledge of sexual and reproductive health, experiences and knowledge of gender-based violence, satisfaction with services and the dynamics of empowerment of Sex Workers (SW).
Specific objectives
The consultancy should enable the following:
- Measure the evolution of key project indicators (baseline vs endline);
- To assess the achievement of the specific objectives and expected results of the DESPS project;
- Analyze the variations observed according to areas, profiles and vulnerabilities;
- Identify the factors explaining the progress, stagnation or setbacks observed;
- Formulate operational and strategic recommendations based on the data produced.
The survey will need to be based on a methodology rigorously comparable to that of the baseline, combining quantitative and qualitative components.
Maximum budget available: 15,000 EUR including tax.
Desired profile
- Proven experience (at least 3 similar missions) in endline evaluations, comparative surveys or project monitoring and evaluation.
- Proficiency in quantitative methods (questionnaires, sampling, comparative statistical analysis) and qualitative methods (interviews, focus groups, thematic analysis).
- Essential experience working with key populations, particularly Sex Workers (SW), including conducting sensitive interviews and adhering to ethical and safety principles.
- Solid knowledge in SRHR, HIV/STIs, GBV, Harm Reduction (HR) and human rights.
- Experience in analyzing partnership dynamics and projects implemented with CSOs and public institutions.
- Excellent writing and summarizing skills in French.
- Prior experience in Madagascar or the Indian Ocean region is appreciated.
- Fluency in French; fluency in Malagasy or collaboration with a qualified interpreter strongly recommended.

- Deadline for expressing interest: January 2, 2026 at midnight, Paris time
- Deadline for a complete proposal: January 24, 2026 at 12:00 PM, Paris time
- Preparation phase: February 2nd to February 11th, 2026 (10 days)
- Estimated duration of the field phase: February 8 to February 28, 2026 (3 weeks)
- Report drafting phase: March 1 to March 22, 2026, including drafting the provisional report, review and feedback from the steering committee, and finalization of the report by taking into account the committee’s comments (3 weeks)
- Return: March 23 to March 25, 2026 (3 days)

Service providers wishing to express their interest are invited to provide the following two items:
- Up-to-date curriculum vitae (CV) of no more than 3 pages for the members of the proposed team, specifying where applicable the consulting firm to which each member of the team is attached.
- A letter of interest of a maximum of three (3) pages indicating: experience in similar missions, at least two (2) references concerning the execution of similar contracts, dates of availability to carry out the consultancy, etc.
An expression of interest will only be considered complete if it includes these two elements. A maximum of four candidates will be shortlisted and invited to submit a full proposal in response to the terms of reference that will be sent to them.
