Doctors Without Borders, an international humanitarian medical association created in 1971, provides medical assistance to populations whose lives are threatened: mainly in cases of armed conflict, but also epidemics, pandemics, natural disasters or exclusion from care.

Diabetes specialist responsible for evaluating and contributing to the improvement of diabetes care in MSF projects in Carnot and Bria, Central African Republic.

Context

In 2009, MSF launched an emergency project in response to a severe nutritional crisis in the Central African Republic. In 2010, following a retrospective mortality survey, MSF resumed HIV/TB care at the Carnot District Hospital. Over the years, the project has focused on improving the quality of care, notably through the establishment of a follow-up clinic and the integration of adult outpatient services.

MSF provides long-term follow-up for adults and children living with HIV and non-communicable diseases (NCDs), including diabetes, epilepsy, asthma, and kidney failure. This strategy has enabled the detection of 1,572 cases of NCDs , including 82 cases of type 1 diabetes .

Since 2024, the project “A Pathway to Patient Empowerment by Transforming Diabetes Care in MSF” has aimed to improve the standard of care for people living with type 1 diabetes (T1D) at Carnot Hospital.
The new care package focuses on empowering patients by improving access to insulin self-injection and home blood glucose self-monitoring.

Staff have received dedicated training in the clinical management of type 1 diabetes, patient education is now more formalized, and patients are empowered. Instead of coming to the hospital twice a day for a biphasic insulin injection, patients now receive a clay pot for home storage, a blood glucose meter, and a month’s supply of insulin pens for self-administration.

The next phase involves transitioning patients from a regimen based on human NPH insulin or biphasic insulin to a basal-bolus regimen with analog insulins.

Mission

Support the introduction of new insulin analogs and facilitate the transition to the basal-bolus regimen. Support therapeutic education activities, particularly for learning the new basal-bolus insulin regimen.

Main responsibilities

Provide refresher training on type 1 and type 2 diabetes for clinical staff (outpatient and inpatient). Provide technical support during consultations and inpatient activities.

To examine the pathway between hospitalization (IPD) and outpatient consultations (OPD) of diabetic patients, and to evaluate the transfer mechanisms.

Conduct patient care reviews.

To train and support staff on consultation skills for the management of diabetes (including type 2 diabetes without insulin), including: collection of history, analysis of results of self-monitoring of blood glucose, screening for complications, adjustment of doses of analog insulin according to the results.

Evaluate the appropriateness of treatments prescribed to patients (including the use of insulin).

Train and support Clinic staff in adjusting insulin according to MSF Guidelines, particularly during the transition of patients from the basal-bolus regimen (Human NPH or biphasic insulin to the basal-bolus insulin analogue regimen).

Desired profile

  • Doctor, specialist or with extensive experience in diabetes and/or endocrinology.
  • Solid clinical experience in the management of diabetes in adults (consultation and hospitalization). Experience in pediatrics is an asset.
  • Minimum 3 years of clinical experience.
  • Last clinical experience less than a year ago.
  • Training in tropical medicine or experience in a limited-resource setting is preferred.
  • Experience in training, supervision and capacity building.
  • Good command of French (spoken and written).
  • Flexibility and ability to work in a team.

Employment conditions

Status: 2-month fixed-term contract, full-time, based in Carnot, Central African Republic. Field position, salary level 9 according to the MSF field salary scale and based on seniority and relevant experience for this position. Health insurance, travel, accommodation, and living expenses (per diem) covered by MSF. Leave according to MSF’s break policy.
