Point-of-Care Ultrasound (POCUS) Specialist (M/F) At Doctors Without Borders

Doctors Without Borders, an international humanitarian medical association created in 1971, provides medical assistance to populations whose lives are threatened: mainly in cases of armed conflict, but also epidemics, pandemics, natural disasters or exclusion from care.

The designated contact person participates in defining the association’s medical and operational policies and strategy by providing knowledge of their specialty and support in its implementation.

General context

The integration of point-of-care ultrasound (POCUS) within MSF aims to ensure that clinicians, whether locally recruited or part of the international mobile staff, have the necessary knowledge, guidance, and access to use this diagnostic tool appropriately and selectively within the overall care of patients in remote and resource-constrained environments. There
are currently regional POCUS coordinators in the DRC, Yemen, Bangladesh, CAR, and South Sudan, working across these regions to address their specific POCUS needs.

Following the TIC POCUS, which focused on expanding, scaling up, and integrating point-of-care ultrasound (POCUS) into MSF operations, particularly by involving national clinical staff, MSF France has decided to continue this initiative by recruiting a dedicated focal point within the medical department. This position will provide high-quality hands-on training, support the ongoing implementation of POCUS, and focus on building local capacity within OCP projects, with potential support to other operational centers as needed and feasible.

To achieve these goals, the POCUS Coordinator will continue to work closely with MSF’s OCP medical coordinators, operational units, projects, and other intersectoral POCUS coordinators and regional managers to support POCUS implementation. The Coordinator will develop POCUS strategies tailored to the mission’s needs and coordinate POCUS requirements across the section.

I. PLACE IN THE ORGANIZATION

The POCUS Lead reports hierarchically to the Head of the Diagnostics Department and functionally to the Clinical Integration and Innovation Coordinator (POCUS Project Coordinator) and the Deputy Medical Director of OCP. They work closely with the clinical leads in the medical department who focus on improving clinical reasoning. They will coordinate the POCUS program within the Diagnostic Imaging Working Group, through the Clinical Integration and Innovation Coordinator.

In the field, the Referent operates according to a matrix system: hierarchical and functional responsibility remains at headquarters, while operational coordination and contextual alignment are ensured by the mission management team. Technical guidance remains the responsibility of the medical department.

II. JOB OBJECTIVES

The OCP POCUS Coordinator will be responsible for:

  • Develop, execute and evaluate the implementation of ongoing POCUS training programs within OCP, with specific strategies to strengthen local POCUS teaching capacities.
  • Acting as an ultrasound expert within the medical department and with project staff at the Paris Operational Centre, possessing a general knowledge of the principles of POCUS within MSF and being able to train CO members on the philosophy and implementation of the program.
  • Collaborating with the Diagnostic Imaging Working Group, the POCUS project team, the medical department and other sections to improve the use and impact of POCUS.
  • Ensure overall supervision of all POCUS activities for MSF France and support intersectoral coordination with the wider POCUS program team.
  • To serve as an ultrasound expert for MSF France for relevant work with MSF Logistics, OCP Biomeds and OCP Telemedicine, in coordination with the POCUS team.

III. MAIN RESPONSIBILITIES

  • Technical Lead for POCUS : Provide OCP-specific and intersectoral coordination and support as the technical lead. Key tasks include annual strategic mapping, budgeting, and serving as the primary contact for MSF Logistics/Biomed/MedRef/Cells/Telemedicine/Medical supervisors.
    • Promote best practices in the use of ultrasound and strengthen clinical reasoning to ensure appropriate indication, interpretation and integration of POCUS results in patient care.
    • Serve as the technical expert for all telemedicine cases related to ultrasound within OCP projects. Liaise with telemedicine experts and clinical case coordinators regarding specific ultrasound cases within OCP. Actively report any necessary changes to improve the program to MSF and the telemedicine team.
    • Promote the appropriate use of the telemedicine platform among local medical teams and, where appropriate, train clinicians in the use of MSF’s telemedicine platform for ultrasound cases.
    • Advise and directly supervise the validation, purchase, maintenance, daily use and repair/replacement of ultrasound equipment with international procurement centers, biomedical teams and field teams for all relevant MSF projects within OCP.
    • Identify solutions and improvements in MSF’s biomedical supply, procurement and support with regard to ultrasound equipment, through reviews of ultrasound devices, direct feedback to the biomedical working group, development of educational materials and training of relevant staff.
    • Serve as an OCP consultant on the POCUS. Participate in internal medical or operational meetings, as needed, and liaise with key stakeholders of this CO (medical units, cell, referents, BioMed, HR, etc.).
    • Provide quarterly updates on OCP POCUS activities to relevant training partners, platforms and line managers to ensure integration within the MSF movement with other clinical training programs and priorities.
  • POCUS implementation : analyze new/ongoing training requests, organize training, inform and support external POCUS trainers and IMS field staff, collaborate closely with existing POCUS users in the field, control quality and ensure longitudinal follow-up.
    • Throughout the year, the MSF OCP POCUS Coordinator will have a highly mobile role, with frequent field visits dedicated to practical POCUS training and capacity-building activities. The objective is to provide POCUS training for new OCP projects, consolidate existing skills through several training cycles within projects where POCUS is already being implemented, and place particular emphasis on developing local capacity to facilitate POCUS in areas with limited access.
    • Provide practical field implementations whenever possible, combining technical expertise in ultrasound (image acquisition and interpretation) with medical/clinical expertise (development of appropriate clinical care based on ultrasound findings).
    • Directly assess the relevance of implementing POCUS in MSF projects, advise teams on the scope of training, adapt and develop a program tailored to the project, and then coordinate field deployment according to the specific context.
    • To serve as a technical mentor for all POCUS facilitators deployed in the field under their responsibility, including for technical briefings, ongoing support and debriefings.
    • Liaise with the POCUS pool manager for the recruitment, assignment and scheduling of facilitators.
  • Quality, integration, sustainability : the Referent.ea’s role is to assess the quality of POCUS use within MSF, its integration into clinical care, and the sustainability of POCUS within OCP projects. Identifying, developing, and mentoring future leaders, super-users, and facilitators is a central element of this role.
    • The POCUS Facilitator’s role is to build local and regional capacity for facilitating POCUS. Their main tasks include identifying key clinicians for facilitator training, co-facilitation, and super-users within projects, and working with POCUS-trained sites to create teaching opportunities for these clinicians. Ultimately, developing country-level POCUS leaders and decentralized leadership opportunities should be priorities.
    • Develop POCUS focal points within the project and support them in the daily clinical use of POCUS in MSF OCP projects, by identifying and mitigating difficulties.
    • Advise and support medical coordinators and project teams towards longitudinal, sustainable and responsible integration of high-quality POCUS use in all relevant MSF OCP projects, including budget planning and annual training.
    • To directly support, advise, and supervise field teams in clinical management using ultrasound. This includes providing direct feedback for the management of complex clinical cases, connecting them with external telemedicine experts, and ensuring additional context-specific training on the Point of Care (POCUS) with longitudinal implementation for all OCs in the region under their responsibility.
    • Contribute to the ongoing ultrasound program within MSF, including by appropriately integrating POCUS into clinical guidelines and providing up-to-date, evidence-based information to clinical care working groups within MSF. Liaise with working groups, particularly those focused on pediatrics, surgery, and critical care.
    • To monitor the transformational impact of POCUS through high-quality implementation of ultrasound in order to change clinical care at the project, regional and international levels, and to identify areas for improvement.

Profile

Formation

  • Licensed and approved physician
  • A POCUS competency certificate issued by a recognized academic institution (or an equivalent MSF certificate) is required.

Experience

  • At least 3 years of professional experience with extensive experience in performing and interpreting ultrasounds in children and adults. Experience teaching point-of-care ultrasound to clinicians.
  • Clinical experience in several of the following areas: pediatrics, obstetrics, infectious diseases (TB/HIV) and adult diseases.
  • Basic expertise in ultrasound specific to several fields (cardiac, pulmonary, obstetric), with a willingness to learn/acquire skills in several other fields if necessary.

LANGUAGES

  • Fluency in English or French is required: minimum level = B2 in both languages. Proficiency in another language is desirable, particularly one related to one or more countries involved in the project.

Status and conditions

Status: 12-month fixed-term contract, part-time (50%). Management position based in Paris, requiring significant mobility with frequent travel to field sites and other MSF medical departments. Teleworking according to MSF’s rules (2 days/week).

Conditions: â‚¬56,100 gross annual salary over 13 months (pro-rated for part-time work). Supplementary health insurance fully covered by Doctors Without Borders. Meal vouchers with a face value of €12 (60% covered by Doctors Without Borders). 50% reimbursement of public transport pass.
