ORGANIZATION OVERVIEW
Doctors of the World France (MdM-F) is a medical advocacy organization dedicated to international solidarity, founded in 1980. For over forty years, the organization has worked to promote universal access to healthcare for the most vulnerable populations, both in France and internationally. Its work is based on a dual mission: to provide healthcare to people excluded from healthcare systems and to bear witness to the obstacles that limit access to care in order to have a lasting influence on public health policies.
Doctors of the World is an independent, committed, international non-governmental organization that goes beyond medical assistance by defending human rights, human dignity, and social justice. It works particularly with marginalized and vulnerable populations, or those affected by humanitarian crises, conflicts, and disasters.
Present in more than thirty countries around the world, MdM-F focuses its interventions around six strategic priority areas: humanitarian spaces and emergency situations, sexual and reproductive health and rights, harm reduction, migration, exile, rights and health, environmental health and the strengthening of health systems.
In the Democratic Republic of Congo (DRC), Doctors of the World (MdM-F) has been present since 1994. The organization implements humanitarian and development programs aimed at improving access to healthcare, strengthening health systems, and protecting vulnerable populations affected by recurring humanitarian crises. Today, MdM-F operates primarily in the provinces of Kinshasa, Tanganyika, and North Kivu (Goma), through several projects focused on health, protection, and the promotion of fundamental rights.
ORIGIN OF THE REQUEST
Between August 2022 and December 2025, MdM-F implemented a Health and Environment Program in Kinshasa, within the framework of a Program Agreement funded in collaboration with the French Development Agency. The program aimed to reduce the health and environmental risks to which market gardeners are exposed, particularly those related to the inappropriate use of chemical pesticides.
In 2026, additional funding from MdM-F’s unallocated resources made it possible to continue certain activities in order to consolidate achievements, strengthen the capacities of actors and promote the sustainability of interventions.
In this context, MdM-F wishes to conduct an external evaluation of the program to assess its relevance, coherence, effectiveness, efficiency, impact, and sustainability. The evaluation will also document best practices, challenges encountered, and lessons learned, in order to guide the design of future environmental health interventions.
BRIEF PROGRAM OVERVIEW
The Health and Environment Program aims to improve the health of market gardeners, their dependents, and consumers, while contributing to the protection of ecosystems in Kinshasa’s market garden sites. It specifically addresses the health, occupational, and environmental risks associated with the inappropriate use of chemical pesticides.
More specifically, the program seeks to improve access for market gardeners and their dependents to health services and health insurance, to consolidate community initiatives in favor of sustainable agriculture and to support advocacy for a regulatory framework that protects health and the environment.
EXPECTED RESULTS
The program pursues three main outcomes:
- Improved access to healthcare, through the strengthening of three health centers, access to health insurance and consideration of occupational risks related to market gardening;
- Strengthening the capacities of market gardening communities, with a view to adopting sustainable agricultural practices that protect the health of market gardeners, their households, consumers and ecosystems;
- Maintaining dialogue with decision-makers, in order to promote the adoption of regulatory texts protecting human health and the environment.
MAIN AREAS OF INTERVENTION
- Health and access to care component
This component mobilizes the relevant national programs, including the National Occupational Health Program, the National Reproductive Health Program and the National Program for the Promotion of Health Mutuals.
It also includes the central offices of the Masina 1, Masina 2 and Nsele health zones, the APRM, Kimbela and Mapamboli health centers, the Lisungi health mutual and the King Baudouin General Referral Hospital of Masina.
It targets approximately 3,000 beneficiaries, including 500 market gardeners and 2,500 dependents.
- Community and agroecological component
Implemented with Caritas Development Kinshasa, this component supports 500 market gardeners, grouped within 14 associations and spread across the sites of Masina Rail, Tshuenge and N’djili Brasserie.
It aims to promote the transition from risky agricultural practices to integrated and agroecological practices, which are more respectful of health and the environment.
- Advocacy component
This component is supported by the Congo Agroecological Network, a consultation framework bringing together actors committed to the promotion of agroecology.
It aims to strengthen dialogue with decision-makers and to support the adoption of regulatory measures relating to pesticide management as well as the protection of human and environmental health.
The main activities of the program include:
- Training for service providers in occupational health and safety as well as on other health topics;
- The rehabilitation, equipping and provision of medicines to partner health centers;
- Organising occupational health and safety consultations for market gardeners;
- Vaccination of market gardeners against tetanus;
- The affiliation of beneficiaries to a health insurance company and their referral to a partner hospital;
- Training market gardeners in agroecology, in the production of biopesticides and in the management of farmer field schools;
- The distribution of biological inputs, personal protective equipment, small farming equipment and seeds;
- Raising awareness of sustainable agricultural practices;
- Organising spaces for dialogue on pesticide management regulations in the DRC;
- Participation in national and regional events dedicated to environmental protection and health promotion.
OBJECTIVE AND SCOPE OF THE EVALUATION
The external evaluation aims to assess the relevance, coherence, effectiveness, efficiency, impact and sustainability of the program over the period 2022-2026. It will have to measure the results obtained, document the changes observed among the beneficiaries and produce recommendations for a possible new phase.
The evaluation will cover the three components of the program: access to healthcare, sustainable agriculture, and advocacy. The stakeholders involved are the MdM-F teams, implementing partners, authorities, health facilities, members of the Congo Agroecological Network (RAE), market gardeners, and their households.
EVALUATION AND METHODOLOGICAL QUESTIONS
The evaluation will be structured around four axes: understanding health and environmental risks; relevance and adaptation of intervention methods; community ownership and strengthening of local capacities; as well as partnership, coordination and advocacy.
A mixed-methods approach combining quantitative and qualitative methods is recommended. This will include a literature review, individual interviews, focus group discussions, data collection from beneficiaries, and information triangulation. The analysis will integrate the OECD DAC criteria as well as cross-cutting dimensions related to gender, inclusion, community participation, and sustainability.
REQUIRED SKILLS AND EXPERIENCE
The assignment will be entrusted to a consultant, firm, or team with proven expertise in evaluation, social sciences, public health, or any other relevant field. To guarantee the independence of the evaluation, candidates must not have been directly involved in the design, implementation, or monitoring of the program being evaluated. Any actual or potential conflict of interest must be declared.
1. General Qualifications and Experience
The consultant or team will need to justify:
- Proven experience in evaluating projects or programs, particularly in the health and development sectors;
- A good command of quantitative, qualitative and mixed methods;
- Experience in designing tools for data collection, analysis and triangulation, as well as in formulating operational recommendations;
- Experience in development cooperation, within an NGO or an international solidarity organization;
- Having professional experience in Francophone Africa, with that acquired in the Democratic Republic of Congo being an asset;
- Excellent analytical, synthesis and writing skills in French;
- From experience in evaluating similar programs.
2. Specific technical skills
The consultant or team will need to demonstrate:
- Expertise in public health, community health or environmental health;
- A good understanding of the interactions between health, environment, working conditions and agricultural practices;
- Experience in agronomy or occupational health and safety is considered an asset;
- A mastery of approaches relating to gender, inclusion, participation and community empowerment;
- An ability to analyze the relevance, coherence, effectiveness, efficiency, effects and sustainability of interventions;
- A good understanding of the ethical principles of evaluation, including informed consent, confidentiality, data protection and the principle of ‘do no harm’.
In the case of a team, skills can be distributed among its members. The proposal must specify the team composition, the responsibilities of each expert, and the complementarity of the proposed profiles.
DELIVERABLES, TIMELINE AND APPLICATION PROCEDURES
The selected consultant or firm will be required to produce a scoping document, a draft report, a final evaluation report, a summary report, and the necessary presentation materials. The assignment will take place between October and December 2026, and all deliverables must be completed by December 15, 2026.
